Senior Clinical Pharmacist (General Practice)
Innovations in Primary Care Ltd (IPC Ltd) is a GP Federation that provides a number of general practice medical services including extensive nurse training and same day GP appointments via local Minor Injury Assessment and Minor Illness (MIAMI) services. As a result of a successful bid as part of the NHS England Clinical Pharmacists in General Practice Phase 2 programme, IPC Ltd. is establishing a General Practice clinical pharmacy service in Worthing and Adur, West Sussex. We wish to adopt an integrated approach that addresses interface issues, reduces gaps in service and promotes quality services for patients. IPC Ltd. are seeking a senior clinical practice pharmacist to become an integral part of the General Practice team.
This is an exciting opportunity to develop a new role within Primary Care. You will work as part of a multidisciplinary team to manage repeat prescription authorisations and reauthorisation, acute prescription requests, management of medicines on transfer of care, and systems for safer prescribing and optimisation. Alongside business as usual responsibilities, some of your work will be project based, to ensure you have an interesting and developmental role.
The successful candidate will have a post-graduate diploma or higher degree in clinical pharmacy, will be an independent prescriber and have a minimum of five years post-qualification experience. They will perform face-to face medication reviews of patients with polypharmacy - especially for older people, people resident in care homes and those with multiple co-morbidities. The successful candidate will provide leadership on quality improvement and clinical audit, as well as managing some aspects of the Quality and Outcomes Framework and the local General Practice Transformation Agreement scheme. We foresee the senior clinical pharmacist providing an important role in the development and support of clinical pharmacists which we are also currently recruiting to.
