In 2009 Tower Hamlets PCT embarked on a major investment programme in primary care which resulted in one of the most deprived Boroughs in London being ranked within the top three CCG areas nationally for 10 of the 65 QOF clinical indicators. Key to this transformation was the creation of 8 networks comprising 4 or 5 neighbouring practices working collaboratively to improve patient care. Tower Hamlets GP Care Group (THGPCG) was formed in 2013 to enable practices to participate at scale to deliver primary and community care services.
The Care Group was incorporated as a Community Interest Company limited by shares in September 2014, with each of the 37 practices in Tower Hamlets owning a share. In April 2016, the Care Group commenced a contract to provide health visiting services to the Borough, taking on 140 staff from Bart's Health. From April 2017, the THGPCG in alliance with Barts Health and East London Foundation Trust has provided community health services to the Borough. It has expanded to provide direct management responsibility for the GP Out of Hours Service, single point of access, health advocacy and interpreting services as well as the patient experience team.
In this context THGPCG is now recruiting 10 pharmacists to work in each of the Networks to add to the primary care skill mix and improve access to clinical pharmacy services in the Borough.
This is an exciting opportunity to be involved in an innovative NHS England development programme to integrate the clinical skills of a pharmacist into the general practice team. We have eight roles to fill:
- Clinical Pharmacist – Eight full-time

Minimum of 2 years post registration experience
Experience of working in a clinical setting in a multidisciplinary team

Clinical Practice Based Pharmacist
Responsible to: Network Lead GP
Accountable to: Clinical Director, Tower Hamlets Care Group
Base: Tower Hamlets
Salary: £31,696 per annum, depending on experience
The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team of pharmacists in general practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage and mentor them.
The post holder will work as part of a multi-disciplinary team in a patient-facing role. The post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice(s).
The post holder will provide clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies and so requires motivation and passion to deliver excellent service within general practice.
The post holder will be supported to develop their role to become a non-medical prescriber.
