Medway & Swale Primary Care Networks, in conjunction with Medway NHS Foundation Trust are looking for enthusiastic and motivated individuals who want to be involved in defining the role of the PCN Pharmacist and contribute to our emerging Integrated Care Partnership (ICP) working across Medway and Swale .

Primary Care Networks (PCNs) are a key part of the NHS Long Term Plan, with all general practices being required to be in a network by June 2019, and Clinical Commissioning Groups (CCGs) being required to commit recurrent funding to develop and maintain them.

Under the plans, all general practices will be aligned to a PCN, covering 30,000-50,000 patients, with local Enhanced services funded by CCGs and provided through the new network contracts. The networks will provide the structure and funding for services to be developed locally, in response to the needs of the patients they serve.

Medway and Swale have 10 PCNs: Medway Rainham, Medway South, Medway Peninsula, Strood, Rochester, Sittingbourne East, Sittingbourne West, Sheppey, Gillingham,and Medway Central.

The posts that we are recruiting to are graded Band 7 - 8a and will be hosted by Medway NHS Foundation Trust, who will provide access to specialist services and dedicated support & development.

All health & social care organisations locally are committed to creating a cohesive local Integrated Care Partnership with active networking across the organisations for the PCN pharmacists.

Medway & Swale PCNs care for a diverse group of patients across a mixed urban & rural geography including some areas with significant health challenges and a high proportion of younger patients with multiple morbidities and complex pharmaceutical needs. Our PCNs are recognised for their wide use of multidisciplinary teams to provide healthcare local to our patients

Access to the National Training & Development programme Access to experienced clinical & pharmacy teams in Medway Hospital Support from the North Kent Training Hub Focus on Frailty to participate in a local Pharmacy Network supported by our nationally recognised Frailty Specialist Pharmacist Mental Health Pharmacy support from Kent & Medway Partnership Trust Local training support Peer support network Medway Hospital has a vibrant and progressive Pharmacy Team, embracing continuous Quality Improvement as a national exemplar. The Medway Pharmacy team is committed to the development of supported Pharmacy networks across the ICP
