Job Description
Job Title: Clinical Pharmacist
Normal Place of work:Frome Medical Practice
Accountable to: Lead Pharmacist/ Medicines Management Team, GP & Clinical Lead
Job Purpose:
Frome Medical Practice is a primary care network in its own right with a patient list size of nearly 30,000 patients. As Clinical Pharmacist you will play a key role in our medicines management team. The team consists of a full time Pharmacist, a full-time Pharmacy Technician, and 3 prescription navigators. The role will ensure medicines management optimisation and safety. It will include multidisciplinary working with other clinical teams.
Main Duties and Responsibilities
Medication Reviews
Undertake clinical medication reviews with patients and produce recommendations for GPs and Nurse/Nurse Practitioners on prescribing and monitoring.
This will include ordering relevant monitoring tests as appropriate.
This will include oversight of reducing polypharmacy, which is one our key outcomes as a green impact practice.
Repeat Prescribing
Review, propose and action any necessary changes to the practices repeat prescribing and medication management policies
Working with our full-time pharmacist and Pharmacy Technician and prescription navigators to manage the repeat prescribing re-authorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates; make necessary changes as a non-medical Prescriber, and ensure patients are booked in for necessary monitoring tests where required.
Medicines Management following Discharge from Hospital as part of the From Hub Team
To reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
Undertake clinical medication reviews with patients and produce recommendations for senior clinical pharmacist, nurses and/or GP on prescribing and monitoring.

Person Specification: Clinical Pharmacist
Qualifications Professional Registration & Training
Registration with the GPhC
Mendip of Royal Pharmaceutical Society
Degree in Pharmacy
Non-medical prescriber or working towards
Experience
Min of 2 years post qualifying experience
Experience of planning, management, monitoring, advising and reviewing general medicine optimisation issues in core areas for long term conditions
Skills and Abilities
Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to the GP when appropriate
Able to work under pressure to meet deadlines
Able to work independently and as part of a team
Provide timely reports and audits as required
Excellent communication skills
