An exciting opportunity has arisen for two Clinical Pharmacist to join the West Berkshire Rural Primary Care Network. We are looking for motivated and forward-thinking Clinical Pharmacist’s; one will be based at Lambourn Surgery and the other at Kintbury & Woolton Hill Surgery.

The Clinical Pharmacist is a patient–facing role with plenty of opportunities to utilise your clinical skills to improve the health of the local community; this will include developing medicines optimisation services in our practices. The successful candidates will have a role in managing long-term conditions, supporting hospital discharge prescribing arrangements, providing specific advice for those on multiple medications, clinical audits to improve patient care and supporting the GP practices involved to develop the clinical pharmacist role; clinical supervision and mentorship will be provided to help support the development of the role.

We are seeking candidates who are enthusiastic and adaptable to change, with excellent clinical skills and a good level of competence in their use of IT. Candidates should exhibit good organisational skills and ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families and carers.

We welcome applicants from both entry and advanced levels; the latter will be an Independent Prescriber, performing face-to- face medication reviews of patients with poly-pharmacy, especially for older people, including residents in care homes and those with multiple co-morbidities.  The successful candidates will provide leadership on quality improvement and clinical audits, as well as managing some aspects of the practice’s Quality and Outcomes Framework (QOF).

Qualifications required for these positions

  • Master’s Degree in Pharmacy (MPharm) or equivalent
  • Registered with GPhC
  • Member of RPS
  • Specialist knowledge through a Postgraduate Diploma and qualifications (e.g. clinical, community, therapeutics) or equivalent
  • Evidence of recent and relevant Continuing Professional Development
  • Independent prescriber status or a commitment to undertake the course, for which support will be provided.
