Key Responsibilities: General Pharmacist Duties Perform face-to-face medication review of patients with polypharmacy especially for older people, people in residential care homes and those with multiple co- morbidities Patient facing / over the phone medicine support. Ward experience needed The successful candidate will hold a suitable qualification and GpHC registration and will have experience working within the NHS or Hospital environment. They must have experience/awareness of the breadth of common acute and long-term conditions that are

likely to be seen in a general medical practiceCandidates must hold an independent prescribing qualification.
