Mediplacements are looking to recruit a band 7 Clinical Pharmacist to work with a client of ours based in London.The pay for this position is £27-£32 per hour, and we are looking for someone who can start as soon as possible for an ongoing duration.The role involves supporting the Triage and Assessment team with the safe transfer of care around medicines in the community where district nurses support onward medicines administration in the domiciliary setting. The pharmacist will be based within the Triage
and Assessment team and will work closely with secondary and primary care partners to triage and deliver seamless, safe and effective care for service users under the care of the Community Health Service. Experienced clinical pharmacist who has worked in a
hospital setting• Has a Postgraduate diploma/certificate in clinical pharmacy• Aware of the issues faced at the acute and GP interface with regards to the transfer of information around medicines• Well experienced with high quality medicines reconciliation and review• Must be able to conduct home visits with the triage and assessment team where required"Successful candidates must hold an MPharm degree, be GPHC registered and have experience working as a clinical trials pharmacist.When you work with Mediplacements, you will have access to a range of benefits that will support you in your role.
