Job Summary:
We are seeking a highly skilled and dedicated Pharmacist based in West Midlands. The successful candidate will work within our multidisciplinary team to provide comprehensive pharmaceutical care. This role involves ensuring the safe and effective use of medications, providing expert advice to healthcare professionals, and improving patient outcomes through medication management and optimisation.
Key Responsibilities:
* Medication Management: Oversee the preparation, dispensing, and safe administration of medications, ensuring compliance with legal and professional standards.
* Clinical Services: Provide expert clinical advice on the safe and effective use of medications, including conducting medication reviews and supporting prescribing decisions.
* Patient Care: Conduct thorough assessments of patients’ medication regimens, identifying and resolving issues related to drug interactions, contraindications, and side effects.
* Education and Training: Educate patients, caregivers, and healthcare staff on medication management, proper administration, and potential side effects.
* Multidisciplinary Collaboration: Collaborate with a multidisciplinary team, including doctors, nurses, and other healthcare professionals, to ensure comprehensive and integrated patient care.
* Policy and Procedures: Develop, implement, and review pharmacy policies and procedures to enhance the quality of pharmaceutical services.
* Quality Improvement: Participate in quality improvement initiatives, audits, and research projects to advance pharmacy practices and patient care.
* Documentation: Maintain accurate and detailed records of medication therapy management, clinical interventions, and patient outcomes.
* Professional Development: Stay current with advancements in pharmacy practice and participate in continuous professional development activities.
Essential Qualifications and Experience:
* Education: Bachelor’s or Master’s degree in Pharmacy.
* Registration: Current registration with the General Pharmaceutical Council (GPhC).
* Experience: Relevant experience working as a pharmacist, with a strong background in clinical pharmacy.
* Competencies: Proficiency in medication management, clinical assessments, and pharmaceutical care planning.
* Skills: Excellent communication, interpersonal, and organisational skills. Ability to work effectively both independently and as part of a multidisciplinary team.
* Flexibility: Ability to adapt to changing environments and work effectively under pressure.
Desirable Qualifications and Experience:
* Band 8 Candidates: Additional experience and demonstrated competence in leadership roles, complex case management, and supervision of junior staff.
* Specialised Training: Further training or certifications in clinical pharmacy, pharmacotherapy, or a specialized area of practice.
* Professional Development: Commitment to ongoing professional development and contribution to the advancement of pharmacy practice.
