Monday - Friday 8am - 4pm

Job Purpose

To deliver a high-quality Medicines Optimisation Service to high-risk patients on the frailty Hospital @ Home caseload and ensure an integrated approach to the management of medicines and continuity of pharmaceutical care for patients. To provide professional expertise, advice, support and training on policies and practice to health and social care professionals in the community. To contribute to the clinical governance of the Trust through interventions which optimise the management of medicines in all relevant areas of practice.

Main Duties and Responsibilities

Clinical Pharmacy and Medicines Optimisation Service;

To deliver a high-quality Medicines Optimisation Service to improve medicines-related outcomes for high-risk patients within the frailty Hospital @ Home caseload and ensure the safe, cost-effective, and evidence-based use of medicines Work closely with the other Hospital @ Home pharmacists across the trust to support each other and ensure standards and goals are transcended across services. To build relationships with key stakeholders that may use or refer into the service. This includes proactively engaging with the medical teams within surrounding hospitals, clinics, PCNs, GPs etc. Conduct clinical medication reviews with patients in their own homes Ensure proactive liaison with relevant healthcare professionals regarding changes to medication regimes. e.g. regular GP, nursing/ care home, community pharmacist ensuring they have the information they need and that follow-up appointments are clear where necessary Make prescribing recommendations as per local and national guidance to the frailty Hospital @ Home team Where the pharmacist is an Independent Prescriber the pharmacist may prescribe within their competence for patients to improve patient care Case manage patients who are frequently admitted to acute trusts as a result of medicines related problems To support the identification of high-risk patients discharged from hospital and ensure appropriate follow-up To provide professional support and compose patient-agreed medication care plans and communicate these with other health and social care professionals e.g. Community Pharmacists, hospital teams, carers etc. Work with the frailty Hospital @ Home MDT to ensure patients are supported to get the best out of their medicines. Ensure liaison with the regular GP, hospital clinicians, community nurses, carers, family, community pharmacists and care coordinators to ensure an integrated approach to medicines management, with a view to ensuring continuity of pharmaceutical care for patients in different healthcare settings Develop a range of practical solutions to enable patients to maintain their independence to self-administer their medication where possible and improve adherence and concordance Liaise with the North Central London ICB Pharmacists to ensure consistency across local prescribing strategies Work collaboratively with system partners to improve medicine pathways for people on Virtual Wards Ensure that developments are linked in with organisational priorities and Local Delivery Plans including cost-effective prescribing Contribute to agreed meetings within and outside the organisation and represent pharmacy as appropriate Respond to unpredictable work patterns and urgent requests for help by phone or email within a reasonable time frame To deliver training sessions to relevant staff groups and/or patients Identify areas of risk in case management patients relating to medicines in primary care and ensure compliance with medicines legislation To collect appropriate quantifiable data to evaluate the service and identify areas for improvement and development To provide information and advice on the preparation and administration of parenteral drugs Coordinate and participate in antibiotic stewardship Respond to medicines information queries from the greater Community Adult Health Service team, including therapists, community nurses, healthcare assistants, social workers etc. To support the Lead Pharmacist for the Hospital @ Home service in workstreams relating to overall Virtual Hospitals as deemed necessary As an agency we cannot sponsor visa’s so any CVs sent without the correct ‘right to work’ or CVs sent from a ‘WHO Red Listed Country’ will be deleted without response.
