Band 7 Pharmacist - London
Job requirements;
• Start Date: ASAP • End Date: Three months + • Working Pattern: Monday to Friday and weekend shifts as allocated • Shift Pattern: 37.5 hrs per week
ROLE SUMMARY
Be responsible for the clinical pharmacy service provided to a sub-speciality within the Emergency and Integrated Care Division. Review prescriptions for accuracy and legality and identify actual and potential problems. Clinically screen and supply of medicines suitable for use as an inpatient and appropriately labelled for discharge. Monitor and review therapeutic use of medicines Ensure the effective outcomes of treatment with medicines. Monitor patients for potential and actual adverse effects of their medicines. Take steps to ensure patients understand the purpose of their medicines and deal with any practical issues that may prevent the optimal use of their medicines. Support the planning of patient discharge with respect to medication. Ensure safe, clinically effective and cost-effective use of medicines. Demonstrate professional accountability to patients. Support the provision of an efficient, safe and legal dispensing service in the Dispensary as required Promote professional standards in the dispensary by acting as a role model to other pharmacists and technicians Locations: •

The Northwest including, Manchester, Liverpool, Blackpool, Cheshire, Blackburn, Bolton, Morecambe •
The Northeast including, Newcastle, Sunderland & Northumbria •

North & South Yorkshire •
East and West Midlands (Derbyshire, Nottinghamshire & Leicestershire) •
Herford and Worcestershire •
