We are looking for a dynamic and forward-thinking pharmacist to lead the Planned Medicine team on a fixed term basis (12 months). The Principal Pharmacist for Planned Medicine is responsible for overseeing the pharmacy service to the following outpatient specialities: Allergy, Immunology, Dermatology, Rheumatology, Diabetes, and Endocrine. The role involves leading a team of specialist pharmacists to: Develop the pharmacy services to optimise patient care, including implementing national guidance. Ensure the infrastructure is in place for the safe prescribing and monitoring of specialist drugs. Work with colleagues within the Care Group and across the interface to develop, implement and monitor treatment pathways, with a particular focus on high-cost drugs. Drive quality improvement and implement cost saving plans such as biosimilar switches to ensure medicines optimisation opportunities are realised. Deliver pharmacist-led outpatient clinics and MDTs safely and effectively. The post holder will play an active role in developing and monitoring treatment pathways with colleagues across South East London, offering an excellent opportunity for networking. The use of new and advanced therapies is rapidly expanding, and previous experience of service development will be beneficial. You will manage a team of specialist pharmacists working across four hospital sites, and the successful candidate must have excellent communication, organisation, and leadership skills. The post holder leads, delivers, develops, manages, and evaluates clinical pharmacy services. The focus of the post is on ensuring the provision of high quality, safe, comprehensive (clinical, supply, financial, and operational) pharmaceutical services at KCH, in accordance with the priorities set by the Deputy and Associate Chief Pharmacists, Clinical Services. Acting as a specialist role model and as an expert in the clinical field and demonstrating an advanced level of knowledge. Ensuring the safe, efficacious, and cost-effective use of drugs within the Care Group. Developing and implementing guidelines, procedures, and policies to support medicines management in the Care Group. Regular financial reporting, drug usage, and expenditure forecasting. Initiating and coordinating research and development activity within the specialist area. Identifying a vision for the delivery of pharmaceutical services to the Care Group. Clinical Responsibilities To be responsible for the provision of a highly specialised pharmacy service (clinical, dispensing, and supply) to patients within the Care Group. To provide highly specialised evidence-based clinical advice and information (doses, interaction, adverse effects, drug availability, complex dose calculations) to nursing and medical staff (within the Trust and externally) on the management of individual patients, in accordance with legislation. To act as a pharmacist independent prescriber to optimise the care of patients. To participate in consultant ward rounds. To manage and make appropriate referrals. To provide direct high quality of care to outpatients in clinic(s) within specialist area, as required. To follow legal ethical professional and employers codes of conduct. To enhance the quality of patient care. To manage difficult and ambiguous problems. To make decisions with limited information. To act as a clinical role model for staff providing clinical pharmacy services to the Care Group. To demonstrate advanced level of clinical reasoning and judgement. To liaise with other pharmacists, services, and Trusts to ensure efficient transfer of clinical and medicines management issues. To promote and deliver pharmaceutical care specifically for patients within the Care Group and generally for all patients. To contribute to and participate in the clinical/ward pharmacy service. To minimise risk to patients through effective supervision of clinicians, nurses, and patients relating to medicine use. To update and maintain patient medication record in accordance with Trust policy. To demonstrate awareness and commitment to the Trust Clinical governance agenda. To identify trends in drug errors and develop strategies to minimise these. To demonstrate a whole-system patient-focused approach. To contribute to, adhere to, monitor, and assess clinical pharmacy service standards. To provide formal or informal patient/carer education and counselling. Person specification

Qualifications

Registered Pharmacist MRPharmS (1 year competency assessed and examined professional registration) Postgraduate Diploma in Clinical Pharmacy or Pharmacy Practice Registered independent prescriber or willingness to complete Skills and Experience

Expert clinical knowledge and skills relevant to area of practice Demonstrate high organisational skills - identifying, planning, organising, and developing systems to improve service delivery Experience of supervising others, including for research and audit projects Experience of protocol/guideline development Be able to provide highly complex information on both clinical pharmacy and finance issues and impart knowledge to other healthcare professionals (up to and including senior consultant level / regional and national level) Ability to work well under pressure by prioritising and managing time effectively Ability to identify and manage risks Ability to implement and manage change Ability to motivate self and others Be able to use available evidence to make sound judgments in relation to patient-or drug-specific issues Understands national and local priorities Demonstration of advanced practice using evidence mapped to the RPS Advanced Practice Framework Advanced critical appraisal skills IMPORTANT Check your email account regularly as this is how we will communicate with you.
